CMS INSURANCE AGENCY PRIVACY POLICY
CMS Insurance Agency has always strongly believed in the privacy and confidentiality of all the information it collects about you. Recent federal legislation, known as the Gramm-Leach-Blilely Act, requires that we notify you about our privacy policies both at the beginning of our relationship and then again each year. We reserve the right to change our policies or practices at any time. We will notify you of any changes at least annually. We do not share any of the nonpublic personal information we collect about you with anyone except as permitted by law or registration.
We may collect the following nonpublic personal information about you from the following sources:
- Information we received from you on applications or other forms, such as name, address, date of birth, social security number, marital status and health information.
- Information about your transactions with us, our affiliates or others, such as claims history, premium payment history and account balances.
- Information we receive from third parties, such as health care provider, consumer reporting agency, insurance company or data collection agency.
Information We Share With Others
We do not sell nonpublic personal information about our current or former customers with any third parties and we do not share that information with anyone unless necessary to process a transaction, service an account or as otherwise permitted by law.
We may share, as allowed by law, all of the nonpublic personal information we collect to the following third parties:
- Our affiliated companies.
- Third parties that market our services and products and administer and service customer accounts on our behalf.
- Other financial institutions with whom we have joint marketing agreements.
Examples of the companies that we may share information with, as permitted by law, include insurance agents, insurance companies, insurance support organizations, credit reporting agencies, registered broker/dealers, auditors, regulators, transfer agents and reinsurers. Before we share information with anyone, we require each company to provide written assurances that they will protect the privacy and confidentiality of that information and will not share or use the information for any purposes other than those for which they were hired.
We do not share personally identifiable health information unless the customer or applicable law or regulation authorizes further sharing. Under no circumstances do we sell or rent your personally identifiable health information to anyone. We only use this information for purposes of underwriting insurance, administering your policy or claim and other purposes permitted by law, such as disclosures to regulators and in response to subpoenas.
We maintain physical, electronic, and procedural safeguards to protect the privacy of the nonpublic personal information we collect. We restrict access to nonpublic personal information we collect to those employees who need to know that information to provide products or service to you.
